Before you can receive your first check, you must submit to Mark Sweatt a completed W9 form. You can download the form here.

I only need the first page and with the online version you can type your information in and then print and sign. If you fill it out by hand, make sure it is legible.

 

Alternatively, Mark and/or Bob will have forms at the field.

 

Referees, if you do not know your taxpayer ID, please get this information from your guardians.
